LUGAZI DIOCESE seeks to engage qualified persons, to support implementation of the Core DREAMS package among vulnerable and at-risk Adolescent Girls and Young Women (AGY), in Luwero District and implementation for KP program in Nakasongola and Luwero districts.

DREAMS PROGRAM

Contract Tenure: 1 year with a possibility of extension subject to availability of funding.

Position Title: Social Economics Strengthening (SES) OFFICER (1)

Position Location: Luweero District

Position Summary: To provide technical leadership and support in the implementation of Social Economics Strengthening (SES) interventions that result in reduction of vulnerability and sustainable improvement in the economic status and overall health of communities across the district/s of operation. To support sustainable economic growth of the AGYWS and support to access to markets

Key Duties and Responsibilities:

  • The SES officer will provide technical leadership to ensure provision of high-quality SES services by the project and Empower Artisans with current trends and knowledge about competing products on the market.
  • Provide technical support to the CBOs, vocational trainers and artisans to provide high quality products that can compete with the current market.
  • Monitor SES services and meet all set targets through supportive supervision, building their capacity during on-job mentorship, training workshops and other platforms.
  • Identify and introduce high impact innovations to improve efficiency, coverage, and achievement of set targets.
  • Benchmark performance in the assigned thematic area with other implementing mechanisms, to ensure quick translation of lessons learnt and best practices into implementation.
  • Participate in the development, adoption and dissemination of new program policies, guidelines, strategies and innovations to ensure implementation is in alignment with the latest standards.
  • Ensure that the latest SES guidelines and policies are available to and utilized by the project, district and community teams.
  • Support AGYWS access to markets from within their communities and beyond
  • Work with the VSLA trainers to ensure that AGYWS are able to make savings

Position Title: Start Awareness Support and Action (SASA) TRAINER (2)

Position Summary: SASA! is a program developed to address the links between violence against women and HIV/AIDS. Key Responsibilities:

  • Deliver SASA training programs to professionals, including counselors, social workers, and healthcare providers.

Position Title: Roving Councillor (1)

Position Location: Luweero District

Position Summary: DREAMS program aims to create a holistic approach to HIV prevention for AGW, addressing both the health needs and the social factors that contribute to their risk. DREAMS program roving counsellor will act as a bridge between AGYW and essential healthcare services, empowering them to take charge of their health. This will be done through Group counselling, coaching and mentorship of the service providers at the safe spaces; SW and SC will then equipe AGYW with Knowledge and resources to make informed choices about their health and well-being. The Roving counsellor will closely work with the Safe space social workers, Subcounty coordinators and Data officers.

Key Duties and Responsibilities:

Mental Health screening and support;

Areas of concern; Mental Health Screening and

  • support HTS, Referral & Linkages, PrEP,
  • The roving counsellor will closely work with the facility teams & SW Mentor and coach them for Pre-test counselling and post- test counselling. educating AGYW about HIV transmission, prevention methods, and the testing process, while addressing any anxieties they may arise.
  • She will be responsible for supporting AGYWS for Mental Health screening and support with Psyco-therapy
  • PrEP (pre-exposure prophylaxis) is a crucial component of the DREAMS program. It is a medication that can significantly reduce the risk of HIV infection for adolescent girls and young women (AGYW) even if their partner is HIV positive.
  • Therefore, the roving counsellor will support improve uptake through counselling that addresses concerns and empowers AGYW to make informed decisions for successful uptake.
  • The roving counselor will work closely with the SW to create Peer support groups among AGYW that can provide information and support their peers.
  • All barriers to Prep uptake will be documented by the Roving counselor.

Violence presents a significant risk factor for HIV infection.

  • The counselor will provide coaching and mentorship for Screening and Identification: while addressing the link between violence and HIV risk, aiming to create a safer environment for AGYW.
  • The roving counselor and SW will periodically mentor and coach
    Subcounty coordinators, facilitators to identify signs and symptoms of violence during interactions with AGYW. All AGYW at risk because of violence will be counseled by the roving counselor and issues will be documented in the Case notes/ GBV register and or Psychotherapy register (Whatever is relevant)
  • She will work closely with the M&E/ program team to ensure standardized screening tools are used to assess a participant’s experience with violence and Mental health.
  • The roving counselor will support teams to develop safety plans that identify potential risks and strategies for staying safe.
  • Roving Counselor and SW will conduct referrals to shelters or other support services that can be provided if immediate safety is a concern.
  • The roving counselor will provide individual or group therapy to help AGYW cope with the emotional and psychological effects of violence.
  • In cases of severe violence, the roving counselor & SW may connect AGYW with law enforcement or legal aid services to seek justice and protection.

Mental Health

  • Mental health is a critical area addressed by the DREAMS program, considering the significant challenges adolescent girls and young women (AGYW) may face. By offering mental health screening, psychosocial support, and referrals, AGYW are empowered to manage their mental well-being and create a foundation for a healthy life.
    Therefore:
  • The roving counselor will engage the line listed AGYW as referred by SW to identify signs and symptoms of common mental health issues like depression, anxiety, and PTSD (post-traumatic stress disorder).
  • The counselor will ensure that teams use standardized screening tools or questionnaires to assess mental health status.
  • In cases requiring more specialized care, the roving counsellor will connect AGYW with mental health professionals like psychologists or psychiatrists at the health facility within proximity.

Psychosocial Support and Counselling:

  • The roving counselor will provide individual or group counselling sessions that address mental health concerns and BCC.
  • The roving Counselor will address sexual and reproductive health issues, promote healthy relationships, and provide life skills training to help AGYW make informed decisions about their health and well-being.

Reporting

  • Write and submit progress reports to the DREAMS project coordinator on a weekly basis.
  • Roving counsellor will closely work with the SW and data officers to document relevant registers e.g. GBV, HTS & Psychotherapy.
  • The roving counsellors may also be involved in data collection and record keeping for the program for the target specific areas; HTS, Prep, KP, Mental health, GBV and related Psychotherapy.
  • Additionally, The Roving counsellor will work closely with the visiting facility teams and Social Worker to support HIV Testing and Counselling through
  • Providing pre and post-test counselling for HIV testing / Retesting. This involves explaining the test, potential risks, and benefits, and offering emotional support throughout the process.
  • Conduct referrals and linkage to Care:
    If someone tests positive, the roving counsellor and social worker will connect them with healthcare providers who can offer treatment and support services.

Required Education, Knowledge and Experience:

  • Bachelor’s degree in social work and social administration Counseling, or a related field.
    Certificate in Counselling is an added advantage
  • Keen at listening and secretly keeping personal information
  • At least one years’ experience in HIV /AIDS related work
  • Knowledge of Ministry of health and local government systems

Position Title: DREAMS Ambassador (1)

Position Location: Luweero District

Position Summary: The DREAMS

Ambassador will support the implementation of the DREAMS project through mobilization, sensitization, and empower other AGYW through personal experiences, lessons learnt and testimonies in assigned districts to ensure that such girls are Determined, Resilient, AIDS Free, Mentored and Safe.

Key Duties and Responsibilities:

Project workplans

  • In collaboration with the project team, implementing partners and Peers will mobilise, engage, empower AGYW using the community-based approaches to access, utilise social and clinical HIV prevention services.
  • Under guidance from the Project Officer, will develop periodic work plans (Annual, Quarterly, and Monthly) for the implementation of community related project activities in line with the project activities.
  • Work with the Project Officers, Social workers, Community leaders, Sub-County Coordinators and facilitators to track the progress, identify barriers, and design corrective actions to attain project results.
  • Organise peer review meetings in area of coverage to update key stakeholders on project progress and strategies, and proactively seek for opportunities to draw synergies with other on-going interventions.
  • Actively participate in team building activities, Coordination meetings at regular intervals as a way of giving and receiving feedback and enhancement of team functionality.

Partnership support

  • Under oversight from the PO, work closely with the Sub- County Coordinators to manage relationships at sub county and community level to be  able to mobilise and engage vulnerable, AGYW in the assigned districts to remain free from HIV infection.
  • Act as a coordination point for the linkage of the DREAMS activities with other Community programs and partners to ensure collaboration, and integration services.
  • Work as an advocate for the DREAMS program at community and district level using the successes

Participate in the referral system to ensure quality utilisation of the Social and Clinical based services

  • Identification and mobilisation of eligible AGYW/ABYM and linking them to social support (peer) groups and platforms to access HIV prevention services
  • Work with the DREAMS peers in facilities to ensure AGYW access quality services.
  • Work with partner CBOs to maintain innovative and empowering group-based activities to promote retention of AGYWs in HIV prevention services

Key Duties and Responsibilities:

  • Manage the daily operations of the Formal Safe Space, ensuring a safe and supportive environment for clients.
  • Provide individual and group counseling services to clients, using evidence-based approaches.
  • Conduct intake assessments and develop individualized support plans for clients.
  • Collaborate with other teams and organizations to ensure comprehensive support services for clients.
  • Supervise and support a team of social workers, counselors, and support staff.
  • Develop and implement program activities, including life skills training and empowerment programs.

The DREAMS Data Officer will support the M&E manager in design, implementation and review of the project’s monitoring and evaluation interventions, collection of primary Data from the field and in putting into the reporting systems, addressing the M&E capacity gaps identified, track performance and inform programming.

Key Duties and Responsibilities:

  • Data Entry and Maintenance:
  • Accurately enter patient information, medical records, test results, and treatment details into the Health Facility’s electronic information systems.
  • Maintain organized and up-to-date paper-based records as needed.

Data Verification and Cleaning:

Position Title: Drop-in Centre (DIC) Coordinator-Clinical-Co/Nurse (2)

Position Location: Luweero District (1) Nakasongola (1)

Position Summary: The DiC District Coordinator-Clinical-Co/Nurse will be responsible for supervision of District nurse, ensuring KP HTS targets are met, all newly identified clients tested positive are linked into care, supported to start ART and as well supported to virally suppress.

Ensure KPs receive the service-layering package and actively enroll high-risk negative clients on PrEP.

The DIC coordinator will also support in championing and providing an oversight for DIC quality management systems, including good customer care, quality improvement, data capture and use.

Key Duties and Responsibilities:

KRA1: Support the DIC team to ensure children of index clients and sexual partners are all targeted for HTS and DIC targets are met.

  • Coordination and Supervision DIC activities on a regular basis
  • Facilitate advocacy meetings and focus group discussions
  • Develop and monitor the weekly work plan of the DIC
  • Arrange weekly and monthly meetings to identify shortfalls and to evolve corrective measures and plans of action
  • Monitor and replenish commodities and supplies such as Needles and Syringe Kits, condoms, and lubricants daily or as needed
  • Organize waste disposal facilities
  • Mobilize, educate and counsel KPs about HTS and combination prevention services
  • Offer Index testing and social network testing to all KPLHIV in ART care
  • Line list contacts (social, biological children, sexual partners) of KPLHIV (and in ART) who need HIV Testing services (HTS) and HIV status neutral services.
  • Identify and link AGYW in transactional sex on the DREAMS platform.
  • Distribute HIVST and SNS coupon among Key populations, and track and report on self -HIV testing among KP peers and their contacts.
  • •Re-offer Assisted partner notification/ index testing and SNS to KPs as per
    SOP.
  • Follow up referrals to CBOs/CSOs, health facilities and ensure KP clients received the services they were referred for, and the facility community and community facility register is well updated on time.
  • Network with community referral points in the DIC’s catchment area to source support to KP clients in need of different services.
  • Ensure proper archiving and storage of
    DIC Registers and files.
  • Ensure availability of the current guidelines, job aids, SOPs, reporting tools required for running the DIC implementation activities.

Required Education, Knowledge and Experience:

  • Diploma in nursing / Midwifery or in health-related field
  • Certificate in HIV Counseling is an added advantage.
  • Working experience of atleast 2years

Position Title: Drop-in Centre (DIC) Peer Educator (4)

Position Location: Luweero District (2) Nakasongola (2)

Position Summary: The PEER EDUCATORS will support the implementation of the KP project through mobilization, sensitization, and empower other keep population through personal experiences, lessons learnt and testimonies in assigned districts to ensure that such people are Determined, Resilient, AIDS Free, Mentored and Safe.

Key Duties and Responsibilities:

Project workplans

  • In collaboration with the KP team, implementing partners and Peers will mobilise, engage, empower KP using the community-based approaches to access, utilise social and clinical HIV prevention services.
  • Under guidance from the Project Officer, will develop periodic work plans (Annual, Quarterly, and Monthly) for the implementation of community related project activities in line with the project activities.
  • Work with the Project Officers, Social workers, Community leaders, Sub-County Coordinators and facilitators to track the progress, identify barriers, and design corrective actions to attain project results.
  • Organise peer review meetings in area of coverage to update key stakeholders on project progress and strategies, and proactively seek for opportunities to draw synergies with other on-going interventions.

Participate in the referral system to ensure quality utilisation of the Social and Clinical based services

  • Identification and mobilisation of eligible KPs and linking them to social support (peer) groups and platforms to access HIV prevention services
  • Work with the KP peers in facilities to ensure KPs access quality services.
  • Work with partner CBOs to maintain innovative and empowering group-based activities to promote retention of KPs in HIV prevention services

Required Education, Knowledge and Experience

  • Must have completed Senior Four.
  • Additional experience and Education qualifications are an added advantage

Position Title: District KP Field Officer (1)

Position Location: Luweero District

Position Summary:

The Field Officer will work with HAC staff and partners (majorly Govt. Health Facilities and rural communities) to conduct community organizing activities, expand work to new villages and Districts Where need be, collect relevant data, and network with relevant officials.

Key Duties and Responsibilities:

  • Ensure KPs (and their clients) receive KP HIV combination prevention package including HTS and PrEP, and STl screening, testing and syndromic management
  • Support retention, and active follow-up of all KPLHIV in care, and compilation of weekly reports on retention indicators.
  • Work with the ART clinic teams to track
    PLHIV KPs who miss appointment
  • Ensure clients that are non-suppressed are offered IAC and enrolled on the Integrated Community Services
    Delivery Model
  • Follow up and ensure KPs who are enrolled on the KP program are retained and get their scheduled HIV test visits
  • Work closely with KP peers to track clients for PrEP refills to improve retention
  • Actively engage KP peers to ensure prevention commodities are available and accessible to the KPs
  • Follow up PLHIV KPs due for Viral Load draw, for better treatment outcomes
  • Work closely with the different stake holders to mobilize KPs for prevention services for better reach and uptake of services
  • Work closely with the CBOs and KP peers to conduct quarterly mapping of hotspots, and their geo-mapping
  • Conduct meetings with KPs to identity areas of improvement in service delivery
  • Act as a liaison between the IP KP representatives and the key populations, to provide two-way feedback
  • Conduct interviews/RCAs among the KPs and PPs for better uptake and service delivery

Required Education, Knowledge and Experience

  • Bachelor’s degree in social sciences and social administration or related
    Field
  • Leadership certificate as an added advantage

Data Entry and Maintenance:

  • Accurately enter patient information, medical records, test results, and treatment details into the Health Facility’s electronic information systems.
  • Maintain organized and up-to-date paper-based records as needed.

Data Verification and Cleaning:

  • Regularly review and verify accuracy and completeness of data entered in the paper and electronic data collection tools.
  • Identify and rectity discrepancies or errors in the data, collaborating with relevant departments as necessary.
  • Provide weekly data extracts from the electronic medical systems and register entry gaps to guide the site level data day exercises.

Reporting and Documentation:

  • Take lead in generating accurate and timely reports for internal and external stakeholders. Prepare summaries, dashboards, and charts for data-driven decision-making.
  • Ensure timely and reliable submission of required reports in MoH, PEPFAR and Program supported platforms like, SURGE, DHIS2, PIRS, trackers etc.

Data Integrity and Confidentiality:

  • Maintain the highest level of patient data confidentiality and adhere to data protection regulations.
  • Safeguard patient records and information from unauthorized access.
  • Follow established data backup procedures and security protocols for all hard and electronic records.
  • Contribute to the development and enforcement of data security measures.

Collaboration and Communication:

  • Coordinate with healthcare workers, HIV program IP staff, and IT personnel to address data-related needs and challenges.
  • Participate in section meetings and discussions to provide input on data management processes.

Key Duties and Responsibilities:

  • A minimum of a Diploma in Information and Communication Technology, Statistics, Medical Records or any other related field of study.
  • A Degree is of an added advantage.
  • Excellent communication and interpersonal abilities.

  • Ability to speak Fluently in written and spoken English.
  • Fluency in commonly spoken local languages is an added advantage.
  • Ability to work independently as well as part of a team.
