Health Information Assistant
Full-time
Under the guidance and supervision of the Senior Monitoring, Evaluation, Accountability and Learning Officer, the Health Information Assistant will be based at the field-Health Facility.
Duties and Responsibilities:
• Providing smooth flow and access to information through records management for action and decision making.
• Ensure the use of standard MoH/Donor data tools and reporting Forms.

• S/he will record data, provide guidance on data recording, compilation, use, reporting and management.
• S/he will produce and submit Periodic health activity reports and foster high standards and practices for quality data collection and analysis. Enable accountability to clients, partners, and donors.
• Implement and support MEAL system and approaches at the Facility and foster learning.
Monitoring and Evaluation:
• Collaborate with MEAL and non-MEAL staff at the Facility level to ensure that data is collected, compiled, checked for its quality and completeness in compliance with IRC and donors’ standards for data quality.
• Undertake regular analysis of facility data and work closely with program officers and SNR MEAL Officer to facilitate decision-making for real-time program adaptation.
• Produce quality reports and update facility dashboards with key findings that foster learning and decision-making.
• Ensure availability and use of Standard/revised HMIS/HIS data tools at all departments of the health facility.
• Providing guidance on use of the HMIS/HIS data tools to ensure completeness, consistency
• Ensure proper storage/filling of reports/forms in designated places.
• Requisitions for registers/ tools and ensuring that they are in constant supply and proper use.
• Register all patients, classify/tally and summarize the data according to the acceptable reporting formats.
• He/she will participate in data cleaning and DQA activities.

• Promptly provide data on notifiable/integrated disease surveillance and response.
• Provide feedback to the facility staff on health service provision for decision making.
• Imparting knowledge and skills to staff by organizing CME sessions, mentorships, and review meetings on data management.
• Participate in capacity building activities (Trainings, Orientations, Mentorship, meetings etc).
• Adhering to Professional Code of Conduct and Ethics.
• Perform any other duties as assigned by the supervisor.
Accountability:
• Work in close collaboration with the SNR MEAL officer, MEAL Manager and the project team, to support the documentation of client feedback and follow up on issues raised by clients to close the feedback loop.
• Identify, document, and share client feedback to improve services and results for our clients.
• Maintain an updated Client feedback registry/database through routine entry of feedback data into the online client Feedback Registry (CFR).
• Working closely with the SNR MEAL Officer, he/she will lead Facility discussions on development and implementation of action plans resulting from the review and analysis of client feedback data.
Learning:
• Document and share learning through Facility performance reviews, data use meetings – ensure requirements, lessons learned, data trends and best practices for products, events, and other knowledge management activities are clearly documented and shared on a regular basis (monthly), this is through data collection and sharing.
• The incumbent will analyse and present performance data (on facility boards) for key program indicators monthly to inform decision making and learning at facility level.
Communication/Reporting:
• Provide regular feedback to the SNR MEAL Officer on activity implementation at the health facility.
• Prepare and timely submit daily, weekly, and monthly data from all facility departments for making reports.
• Participate in the development and implementation of program work plans.
Minimum Qualifications:
• Diploma in medical records and Health Management Information System from a recognized, licensed, and accredited institution.
• Computer literate in MS Office Applications with at least one year of progressive experience in health record management –Essential.
• Applied Knowledge in use of HMIS/HIS data tools.
• Excellent communication and interpersonal skills.
• Minimum 2-year work experience in a busy health setting.
• Ability to completely record, accurately compile and produce timely reports.
• Ability to work in hardship areas (previous experience in a camp setting will be an added advantage).
• Flexible and able to work with team in a dynamic and diverse environment.
• Good standard of spoken and written English Language.
• Ability to speak any of the languages spoken in the refugee settlement is an added advantage.
